Job summary:

The Health Care Integrator (HCI) Supervisor's primary role is to oversee the daily implementation of the Bridges to Health (B2H) program to meet the primary objective of protecting and promoting the health, safety and well-being of children receiving B2H Waiver Services. In that endeavor, the HCI Supervisor is expected to monitor the work of the Health Care Integrator (HCI) and Waiver Service Providers (WSP) to ensure quality assessment, education, coordination, consultation, stabilization, advocacy and provision of linkages to services for children enrolled in the B2H Waiver Program. The HCI Supervisor will work closely with OCFS and Quality Management staff to ensure the timely provision of service, and adherence to program philosophy, mandates, trainings, and timelines and monitor overall staff performance.
